Yes, you need supports. Use paint-on supports to create flying buttresses that not only keep the icicles adhered to the bed, they keep the tiny arms of the icicles from breaking. I've provided a screenshot of how they should look. I've also provided my PrusaSlicer project files and GCODE for a Prusa MK3S+ printer.
The spiral designs are much more delicate than the lattice. If you don't use water soluble supports, when removing the supports, take your time and be extremely careful - especially for the bottom and the loops, though the loops don't really need supports.
I use just one perimeter everywhere except for the loop and the top of the icicle. I use 2 perimeters there. The lattice ornament will require some infill. I used shape modifiers to add infill only where it was necessary.
I recommend using the Classic perimeter generator, not Arachne (the default in modern slicers).
