Combined gear and spinner, you can choose different ways of rotation.Print the cage, wheels, knobs, the middle axle is made of two parts, glue them. For 5 bearings 608, available in roller skates.

I tried the print with a 0.3mm nozzle, the result is nice, the teeth on the gear are sharp
